Part 3:  Your Agreements

Confidentiality Agreement

As a volunteer with Love INC I understand that I may be exposed to information or materials that are of a confidential nature.  I recognize that Love INC assures confidentiality to all clients.  I will disclose no information regarding a past or present client to any unauthorized person.  I will sign a confidentiality form that will be sent to me from the Love INC office and will return it within 7 days of its receipt.  By signing that form, I agree to protect the confidentiality of all individuals who receive services through Gallatin County Love INC ministry.

Statement of Faith

I have reviewed Love INC's Statement of Christian Faith, the Apostles' Creed, and will sign a form indicating my agreement with this expression of faith.

* Please note: if you are not a member of one of Love INC's partner churches, but you are a member of another Christian church in Gallatin County, we welcome your offer to volunteer.  Since you do not have a Church Contact Person, you must contact the Love INC office for further information before your Talent Tithe processing will be completed, though.  Thank you!
