Volunteer Agreement
All Love INC church volunteers must be a member of a
Love INC partner church. In
addition, each volunteer signs a confidentiality agreement and an
agreement with the Love INC faith statement.
Here is the Love INC volunteer confidentiality
agreements:
"As a volunteer with Love INC you may be exposed
to information or materials that are of a confidential nature.
Love INC assures confidentiality to all clients. There is to be no
information regarding a past or present client disclosed at any time to
any unauthorized person outside your sphere of service (prayer group,
office volunteer, church contact person, or as a congregational
volunteer offering direct services to a client, etc.). By signing
this form you are agreeing to protect the confidentiality of all
individuals who have requested services through this ministry."
In addition, Love INC church volunteers sign their names
below a copy of the Apostles' Creed.
The signed confidentiality agreement and agreement with
the statement of faith are kept on file with the Church
Contact Person from the volunteer's church. The agreements are
to be renewed annually.
